This survey is run by the program leader at Cornerstone House of God to plan a health and fitness effort for the church family.

Who sees your answers: only the program leader (and anyone they add as a co-leader). Answers are never posted, shared with the congregation, or sold.

Name and phone are optional. Leave them blank and your answers are anonymous. If you give them, they are used only to follow up with you about the program.

Health questions are optional and are asked so activities can be made safe for everyone. The leader is not a medical professional and will not give medical advice.

Your phone keeps a copy of your answers in its browser storage so you can pause and resume. Clearing your browser data removes it.

Want your answers removed? Tell the program leader and they will delete them.
